A formal radiology report, including both pathological and biomechanical
findings, diagnostic impressions, recommendations and comments
from STREHLOW RADIOLOGY CONSULTING strengthen your patient’s personal
injury case with beneficial documentation.
Submission Process:
Have your patient carefully read and sign the RADIOLOGY
CONSULTATION form.
Have your office staff completely fill out the RADIOLOGY
CONSULTATION form.
The referring physician then can make note of any
specific concerns or significant previous medical history
on the form.
Place the films and accompanying completed paperwork
into an x-ray mailer or tube, affix our prepaid mailing
label and give the packet to your regular U.S. mail
carrier. They will take the packet from your office
and deliver it to our office.

your films and accompanying completed paperwork into
an x-ray mailer or tube and contact our office for convenient
Tuesday / Thursday door-to-door courier service.
STREHLOW RADIOLOGY CONSULTING will read your films and generate
a formal, written, radiology report. The formal report and your
films will be returned by U.S. Priority mail or by our courier
on the next scheduled visit. At your request or if significant
pathology is discovered, a copy of the report can be faxed to
your office immediately following the transcription of the radiology
report. The radiology consultation form (which contains a physician
lien) will be mailed to the patient’s attorney office by our office.
Our office will work directly with the patient’s attorney regarding
payment for the services rendered. If a patient’s attorney belongs
to a law firm that our office chooses not to work with, we will
work with the referring physician (physician agreement) until
the case settles and then the referring physician will pay for
the services rendered by STREHLOW RADIOLOGY CONSULTING.
