A. STREHLOW RADIOLOGY CONSULTING will bill the referring physician utilizing the $20.00 - $10.00 & $10.00 PHYSICIAN'S PROFESSIONAL COURTESY PAYMENT PLAN. This option is excellent for cash patients and for those patient's whose insurance will not cover outside radiographic consultations.

B. STREHLOW RADIOLOGY CONSULTING will hold a DOCTOR'S LIEN directly with the patient, working in good faith with the patient's attorney. The attorney will pay us for our services from the case'Settlement.

C. STREHLOW RADIOLOGY CONSULTING will bill the patient's AUTO INSURANCE- Personal Injury Protection directly for our services.

D. STREHLOW RADIOLOGY CONSULTING will hold a PHYSICIANS AGREEMENT directly with the referring physician. The referring physician, following the case settlement, will pay us for our services.

E. STREHLOW RADIOLOGY CONSULTING will bill the patient's MEDICAL INSURANCE Company directly using the -26 modifier. Our office will then bill the patient for the appropriate co-pay or deductible payment. When dealing with HMOs & PPOs, we encourage the referring office to pre-authorize our radiology consultation to avoid total denials or large co-pays that become the patient's responsibility.

F. STREHLOW RADIOLOGY CONSULTING will bill the PATIENT directly for our services. Appropriate patient information is required.
